🔹 Skilled Nursing Services

Skilled Nursing is provided by Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) and includes short-term or intermittent care for clients with medical needs that require professional oversight.

Common Skilled Nursing Services:

  • Wound care and dressing changes
     
  • Medication management and injections
     
  • Post-surgical recovery support
     
  • Chronic disease management (e.g., diabetes, CHF, COPD)
     
  • Pain and symptom management
     
  • Tube feedings (G-tube, NG-tube care)
     
  • Foley catheter care
     
  • Ostomy management
     
  • Monitoring vitals and clinical assessments
     
  • Coordination with physicians and discharge planners

 

🔹 Private Duty Nursing (PDN)

PDN is hourly, one-on-one nursing care for individuals with complex medical needs often children or adults who require continuous supervision by a nurse throughout the day or night.

Common Conditions Supported with PDN:

  • Ventilator or BiPAP dependence
     
  • Seizure disorders
     
  • Tracheostomy care
     
  • Total care patients
     
  • Neurological conditions (e.g., cerebral palsy, traumatic brain injury)
     
  • Pediatric medically fragile patients
     
  • Medically complex seniors needing 24/7 oversight
     

PDN Services Include:

  • Around-the-clock nursing (up to 24 hours/day)
     
  • Shift-based skilled care
     
  • Emergency intervention & response
     
  • Family education and caregiver support
